Job Description:
Family Medicine Physician Sonoma County Clinics We are seeking a
dedicated Family Medicine Physician to join our team across four
community clinics in Sonoma County. This full-time position offers
an excellent opportunity to provide comprehensive, patient-centered
care in a diverse and collaborative environment. Key
Responsibilities: Provide primary care services to a diverse
patient population, including preventive care, health education,
diagnosis, and treatment of acute and chronic illnesses. Conduct
thorough patient assessments, develop individualized care plans,
and coordinate ongoing management. Collaborate with clinic staff,
specialists, and other healthcare providers to ensure integrated
and quality patient care. Maintain accurate and detailed medical
records in compliance with regulatory standards. Participate in
quality improvement initiatives and clinical protocols to enhance
patient outcomes. Engage in patient education to promote health and
wellness. Qualifications: Medical degree (MD or DO) from an
accredited institution. Board certification or eligibility in
Family Medicine. Valid medical license to practice in California.
Strong communication and interpersonal skills. Ability to work
effectively in a team-based environment. Prior experience in
outpatient primary care setting preferred. Position Details:
Employment Type: Full-Time (0.8 to 1.0 FTE) Locations: Four clinics
